The Silent Witness Form below is for students, faculty, and staff to report suspicious and/or criminal activity to campus security anonymously.

We understand that people may feel uncomfortable reporting crimes or incidents in person at the security office, therefore we offer this program to anyone who has witnessed, or knows about, a crime committed on the USF campus.It is important that you provide as much detail as possible. We appreciate your assistance in helping us protect the USF campus.
